Trailing space in blue text span in Submit document

When submitting document, the blue text span contains a trailing space, even though the exact span was selected in the original text.

Also, added terms contain a * in the text span. Is this intended functionality? It would be nice to show that the term is added in a different way, perhaps in one of the columns?

Also, it would be nice to be able to hide the ICD-10 and OPCS-4 columns when they're not used.
