Don't add `No title` header if there's already a level 1 header in markdown

After thinking some more about this, I don't feel like I want to implement it. This would require looking at the gist's markdown source, and I personally don't think that the complexity involved in doing so exceeds the benefits. This is under the assumption that I've understood the problem correctly, in which case this could be solved by putting the respective heading in the title field of the gist. Is there anything I'm not seeing?
