Create a "Troubleshooting Guide"

When something does not work as expected, most users don't really know how to go about diagnosing the issue. We already have a "Common Issues" document, but I think it would be very useful to also have a "Trobuleshooting Guide" document, that explains things like:
- how plugins & themes can interfere with WP-CLI and how to skip them
- how config files can interfere with WP-CLI and how to find out which ones are being loaded
- how third-party commands can interfere with WP-CLI and how to find out which ones are loaded and how to skip them
