[Binja] Allow multiple custom hooks per address, edit hooks through option in hook list

Discussed with @lordidiot and agreed that implementing the ability to:

• Create multiple custom hooks on the same address, and
• Edit custom hooks via right-click menu in the hook list
  (similar to what was implemented in #77)

would be useful
