Refresh version release "process" notes

To go in the readme or something...
- Poke about in this repo until happy, commit result
- Make another commit with version number in refresh.s65 incremented, and a new entry in refresh.md indicating that this is the version number now under development
- Update acorn_mos_refreshed repo's submodule reference to the commit made in step 1
- Push
The previous version's section from refresh.md should go in the GitHub release comment
