Upgrading to Wagtail 2.11 broke existing functionality

I recently upgraded to Wagtail 2.11.1 and I think some of the internal JavaScript APIs have changed which have broken this library.

This is the error I see after selecting a snippet model:

It appears that maybe there are now two snippet chooser urls?
