Encounter - codes and class should not be present on wellness encounters

The Encounter state should not require codes or an encounter_class for wellness encounters - actually they will be ignored even if provided because the system starts the wellness encounter, so the fields should be toggled on and off as wellness is set to false/true, respectively.
Addressed in #216
