Is it possible surface will ever work with Etherpad?

I don't see why not. It's a matter of integrating it appropriately. I don't know how etherpad is handling annotations on the data-level though.

They are handled as changesets of operational transforms see: https://github.com/ether/etherpad-lite/wiki/Changeset-Library

Part of the Etherpad 2013 roadmap is native clients so it'd be cool if substance can fill that role.

It's not really Substance-related. :) We do not plan Etherpad integration from our side. However as far as native clients are concerned, Substance Composer Alpha 3 will be ready in a matter of days, and be available for all major platforms (OSX, Windows >7, Ubuntu/Debian).

We're more focussed on the offline editing usecase at the moment. However realtime collab is something that we may want to address in the coming months/years.
