Combine extension and library repos

The library and extension should live in the same repo, it will make managing issues between them easier.
I have now moved all the extension issues into this repo. Next, we'll move the extension source code into this repo and create lib / extension folders.
All the obvious stuff is done here. I bet more paths will need updating at some point, but that can be fixed elsewhere.
