create launch context menu

When opening large Stored Programs there is a delay. To enable STPs to be launched more quickly, a right click context item would help, eg here:
Two more items should be added at the bottom:
- Launch (equivalent to the launch button)
- Launch + Debug (equivalent to the launch button + &_debug=131 in the URL)
