`sasjs init` doc issues

There are a few issues with the default doxygen docs after running a sasjs init:
- there is a reference to a doxygen SVG but it is a PNG that appears in the output
- the banner is not appearing correctly due to a styling issue
- the new_header.html file contains a closing div/body/html but that should be removed (belongs in the footer)
closing as was not reproduceable
