[meta] Bug report form doesn't quote code block

Is there an existing issue for this?

Describe the bug
The form for entering bug reports has a text box for "code snippet". If one just pastes code into that, it will not get code boxed and displays incorrectly. This is a frequent occurrence in issue reports received. If there is a separate box for code, one would expect it to quote automatically with ```python. If that is not possible on Github, we should remove that box and assume that code is included in description.
