TOC and Chapter numbering in PDF version

mainbranch to see if this has already been fixed, in this file:- Yes, it is not implemented.

Description of the problem:
When i print the online rust-lang book (convert it to PDF basically) it doesn't contain chapter numbering or TOC in front page like in normal books which makes it difficult to the reader to navigate through the book. I know [toc] is an open issue rust-lang/mdBook#153 but that issue is not specifically asking for common TOC in PDF version.
Suggested fix:
Add TOC and chapter numbering in online version too like old version .
