Have form fields associate with category heading

Describe the bug
A clear and concise description of what the bug is.
It would be helpful to someone using a screen reader to have the form fields associated with the category heading. So, for example, when a screen reader user hears "Street", they will know it relates to the "Business Information and Address" form on this page.
Recommendation
Add a fieldset and legend around each category and associate the input fields with the category title. See Example 3 on this page: https://www.w3.org/WAI/tutorials/forms/grouping/
