Package outwatch-snabbdom

This project is a fork designed to work with outwatch. We want to make this clear in the readme and also publish a proper package to npm (or github package manager) under a new name. I propose outwatch-snabbdom.
We currently use a git depedency to this repo, but that does not really work well. I also want to clean up some code and remove unneeded code that we do not need. We should still try to keep the core as close to upstream, in order to stay sane when merging updates - but who knows what needs to be done :)
