Create a 'common errors' page

Document common errors that can happen during the review/acceptance and possible solutions.
Example: when the GitHub API is down the acceptance action can fail but the branch is created/merged so an error is posted to the review but the accept command can be reissues until the topic branch for the paper is deleted.
