Keyboard input sometimes has stuck modifier keys after switching to application

Sometimes after I switch to my Onivim window/workspace (with ctrl-alt-arrow), my keyboard input gets corrupted. It seems like one or more of the modifier keys are "stuck". Characters I enter in insert mode appear as rectangles, before and after the cursor. It usually helps to switch to a different file/tab in Onivim and back, but not always.
Sorry for the vague description; I don't know exactly how to describe this.
