Rename regions attribute to continents.

It is more deductive to rename the column countries.region to countries.continent.
The country.subregion does make generic sense and can be kept as it is.
@ronlinet please fell free to fork and do your modification. regarding the large number of people downloading and using ghe package i would be more conservative on backward compatibility.
