Performer and recorder must not have references to PractitionerRole

Issue raised by a supplier Observation performer was returning references to PractitionerRole instead of Org or Practitioner.
Also observed in MedicationRequest recorder which must be Practitioner.

This leaves PractitionerRole which is an optional element as an unreferenced entity but which must itself reference a valid Practitioner.

Had to add a reference count for the Role when consultations or problems are selected.
