The url is currently https://fhir.nhs.uk/STU3/StructureDefinition/CareConnect-GPC-Condition-ProblemHeader-1/_history/1.6

but needs to be amended to

https://fhir.nhs.uk/STU3/StructureDefinition/CareConnect-GPC-ProblemHeader-Condition-1/_history/1.6
