User story
As an applicant, I want to provide my contact information so a caseworker is able to get in touch with me easily to complete my application.
As an applicant, I want to indicate what language I speak so I can work with a caseworker who speaks my language.
As a caseworker, I want an applicant's phone number so I can process their application faster than by mail.

Acceptance criteria
- Verify that the user is surfaced the following optional field forms:
- phone number
- alternate phone number
- preferred language
- Verify that the user can skip all of these questions to ensure compliance with quick submit requirements
- Verify that the user has the ability to translate content into Spanish
- Verify that there is a continue button user can use to progress to the next part of the application
- Verify that there is a back button user can that directs the user to previous part of app
