Provide a way to ignore a file in the `changelog.d` directory (e.g. `README.md`)

It would be great to be able to have a file in the fragments directory which explained what the directory is for and point back to scriv for instructions. Unfortunately a README.md file is automatically included by scriv ATM.
Implemented in ed36613.
This is now available in version 0.12.0
