Custom Monograph edit form

We have a custom form for editing FileSets and need to do the same for Monographs, to enable more sensible field groups other than just required/additional and random besides, among other things.

• start off a basic form for Monographs
• get the cardinality right for fields so there are no unnecessary big empty boxes
• add the solution indicated in #1782 so that the Hyrax form-submission JS enforces values in required fields
• maybe add a style for required fields other than just an asterisk so they are easier to pick out now that they won't all be at the top
• look for inspiration in Hyrax (edit_fields or other) specs and consider writing a spec for the form if possible
• override any instruction values that don't make sense in the context of our metadata:
