Update Istio addon to the latest Maistra

Currently, the Istio addon is based on Maistra 0.2, which is severely outdated. We need to update this to the latest Maistra.
This is fixed by #182 for 0.9. We have the 0.10 release coming out shortly and will follow up with another PR.
