Be killed while falling (but not through fall damage). The camera will sometimes be way above the player model in 3rd person. See #12123.

Unfortunately I can hardly make a video due to Wayland shenanigans. Here are some screenshots though:
This is the same player at the same time, viewed once from an outside player and once from his own perspective: The server stops sending position updates because it deems the player dead, but the player continues falling to the ground locally. When you now click respawn, you will correctly be respawned in the air, but it'll feel like a teleportation to you.
The issue description is probably off though...
