Add an option to add only new subfolders of already selected folders.
I second that. This would be reeeeeaaaaally useful !
I wish I could help out, but I can't understand a thing at this whole XUL mess at the moment... (I mean the Thunderbird extension mechanism of course)

Took me two whole days, greping the source, reading IDL files, and getting lost into the MDN jungle... :-/
I uploaded version 0.2 on both github and addons.mozzilla.org.
Now the updating process is fully automated: the search targets are updated whenever a new folder is added or a virtual folder's properties are edited.
The hardest part was figuring out how to hook into the virtual folder properties panel, but at some point I realized that a script in an overlay has access to all global variables defined in the element it extends, and then I was shocked !
