Formatting: miscellaneous issues

All page numbers taken from the PDF returned by T&F on 2021-03-04:
- p65-66: output of shell command at bottom of p65 shows up on p66.
- p97: output line at top of page should appear on previous page.
- p109: move first line of listing to next page.
- p147: need blank lines before formulas.
- p254: need space before equation.
- p323: the callout box overlaps the title---need to introduce some vertical space.
- p345: replace running section number with "A Solutions" throughout instead of having section numbers (which shouldn't start at 0).
- p470: URL overflows.
- p473: URL overflows.
