Take input data from modal, Like filling form

This modal is good, But I'm not getting, how to get input data on a submitting form in modal ?.
Is there any example then please help.

Hi,

Just put your form/data or anything you want show inside a modal instance, just this, it's only javascript, you can handle it as way you want.
