Add registration form for course manager at course new / edit page

Normally, the system staff uses the course member page to register the course members (including managers), but when creating the course, the staff wants to register the course managers at the same time.
So it is convenient to be able to register course managers also from course edit page.
For the time being, this function only needs to be available to system staffs.

This makes it easier for system administrators to register new courses.
