Reconsider snippets table structure

Delete column master_id
- By removing learner's ability to quote snippet from other learner's worksheet, master_id is unnecessary

#Create intermediate table note_indexes for snippets and notes
- In order to avoid duplicating snippets by the number of learners when creating each learners' worksheet from original worksheet
