Document how to open a PR and follow ups for first time contributors

This is for documenting how to open a PR on GitHub, template usage in the text (to auto close the issue), how to update the changelog file, and also general guidelines for first time contributors on how to follow up on threads that reviewers might open.
Also, link the CLA for first time contributors in this page if they haven't signed yet
