Change log or release history?

Is there any consolidated info on the changes between versions, a change log perhaps?
It would be really nice if the releases were tagged in github and had information about the changes since the last version. And/or there could be a CHANGELOG.md. It's difficult to upgrade if we have to read the diffs of every commit to see what changed!
This is old, but still a very relevant request. This is important for upgrade paths.
