Investigation: queues - random videos being added to the end?

While paying attention to people's gripes with YouTube on Hacker News and r/youtube, I noticed multiple people complaining that when they use the queues feature, they're getting random videos added to the end of their queue when Auto-play is on.
They didn't say if they were using the web version or the app - is there anything we need to do here?
