Move hash to core

What it says on the tin. I can't think of a good reason for the user to not include the hash extension, and doing this will clear up some issues with order of extension inclusion.
Hash is now integrated into core on master. In the move I removed the # link that was visible for older browsers that don't support the History API. If it's something people were absolutely in love with I could make that its own separate extension again in the future.
This is included in the 1.1.0 release.
