AU Core Encounter: Remove MustSupport from Encounter.extension:encounterDescription

Recommend MustSupport is removed from Encounter.extension:encounterDescription in AU Core Encounter. Inclusion of this extension / similar element is premature in AU Core - need to have a discussion around how clinical notes are supported.
