Put QR code on one page in multi page booklet only

Right now the QR code plugin puts the QR code on every single map page in a multi page PDF, but having it on a single page -- probably the front page -- would make more sense than repeating it for every individual page.
Filed in wrong project, is now hholzgra/ocitysmap#40
