Problem

We currently optimise data entry by including optional sections with the advice to skip where not relevant. This saves one interaction ("add X?") and one extra screen.

Improvement

Screens PIT 3,4,5: add "add another PIT" question to screens PIT 2,3,4.
Same for FT, biopsy.
Make informed decision which screens to always show. (PIT 1 and 2?)
