Create a 1.3 branch that uses 1.3 translator

The task for this issue is to create a 1.3 and implement/edit new/existing operators and functionality. Use the following 1.3 spec as a guide: http://cql.hl7.org/2018May/index.html (note the change log and version history)
Closing this in favor of cqframework/clinical_quality_language#1074
