Teams ont included in header column on grading page

In the new version for large table, the student name column is static, but not the teams.
The teams should also be static.
On that same page, the CSV file
- does not include the teams
- has grades noted as 5/20, which is inconvenient. Better add a row indicating "graded over" and indicate "20" then show in each row the actual grade
- does not show the details of the grade break-up
