PR template for contribution

To encourage good habits and encourage norms in contribution, a PR template may come in handy. There are lots of open questions here,
- Can we capture everything we need in one PR template or do we need different ones for different packages?
- What would we need to require in the template? Tests? Docs? Code style check (assuming we have that documented)?
- I haven't made one before, how do they work?
