Instructor signup

Add instructor signup option that prompts for a URL to a departmental web page showing the name and email of the instructor.
Add a page in the admin login that allows to review pending instructor requests.
Remove the old admin dashboard once this is setup.
or... we could simply manage this as instructors getting in touch with the admin of the server, double checking credentials like this, then simply assigning instructor role to their account if the credentials check out.
