Formalize Github Issues and Pull Requests with a template

The main idea is to write a template to these two Github public features. This will formalize each Issue or PR, to help maintainers to clearly understand what are the problems a developer is facing or what changes are being introduced in a PR. Is that way, maintainers can quickly help the developer or have a clear idea of whom should be called to help to solve the issue or review the PR.
