exitFee incentives

exitFee should be predefined or have a different minimum to swap fees. The amount pool creators would gain on joins & exits is insignificant compared to trading fees, so are incentivized to set that at the very minimum. Which is bad for Balancer Labs :)
IIRC it wasn't even going to be configurable. Yeah a near-0 minimum makes no sense for rent seeking =P
