Support applicationProfile and protocolRequest in onlineResource

applicationProfile maps to /metadata/distinfo/storder/digform/digtinfo/formspec in CSDGM. See https://www.geoplatform.gov/wp-content/uploads/2018/05/NGDA_Metadata_Guidelines_v3.pdf. Not sure protocolRequest has a CSDGM mapping.
