Create user button is disabled

I tried using this app but for some reason the "Create" button is disabled so it looks like I can't advance from there:

I initially thought it may have something to do with the "Connecting" indicator at the top right, but I tried in a different browser, and the indicator changed into "Connected", but I still can't click the "Create" button.

Regardless of the reason, the "Create" button should have a tooltip (or small print underneath, visible while it's disabled) that explains why it is currently disabled.
