checkbox to mark tnris.org document upload as SGM notes

a checkbox for tnris.org document uploads has been requested due to the additional request of automating the visualization of gis soutions group meeting notes on the tnris.org frontend. the notes are reflected here - https://tnris.org/geographic-information-office/gis-solutions-group/.
idea is to limit a max number of notes on the frontend to 12.
