Personalized room links modal showing old errors

Describe the bug
Error messages in the personalized room links modal are not cleared on close/open, causing the error to show on all future modals (add/edit)
To Reproduce
Steps to reproduce the behavior:
- Go to the personalized room links tab
- Click on add personalized room link
- Submit the form without entering valid data
- See validation error
- Close modal
- Open modal again
- See validation errors still showing
Expected behavior
The error messages should not be shown after the modal has been closed
