display issues -- some data elements missing or not fully displayed, column width problem

some data elements were either omitted or not displayed fully:
- incident
- identity
- victim
- time
- external id
- affected assets
some of the main item tables had header rows with strings that contained underscores which prevented wrapping and caused the header table cell to be wider than expected. One example of this was Incident's Impact Assessment (it was showing up as "Impact_Assessment").
