Sometimes `/Title Before Cursor` does nothing

How to reproduce the bug:

1. create a new page
2. in a single new block, type a valid url, ex. https://github.com, type /Title Before Cursor following the url and press enter
3. do step 2 for multiple times, then the bug will appear
