Visibility weights aren't being used properly

To my memory, the various implementers of the data-reading trait aren't consistently handling weights. The weights should be read from the data and negated if flagged. Also from memory, the weights aren't being used in calibration. They should be at least used to determine if a visibility is flagged. A weighted average of the visibilities might also improve calibration.
